eReferrals

Please follow the link below to login to our eReferrals portal which will allow you to submit an online one-page referral for our Mental Health Services. Once your referral has been submitted, the team at Windsor Road Private Clinic will be in contact to arrange an assessment of your patient. Your account also allows you to view the status of your submitted referrals. 

To assist you with this process, please have the following patient information at hand: 

• Patient’s name and date of birth
• Diagnosis details
• Referring doctor, requested admission/transfer date, requested assessment date
• Funding details (eg Private Health Insurance, DVA etc)
   

Important.

If you are registering as a new user, you will also require a valid email address and contact phone number to submit referrals online.
